Medical Records Request
Your medical record is more than paperwork, it’s the story of your health. Our Health Information Management (HIM) team works behind the scenes to make sure your providers have accurate, secure, and timely information to guide your care. We’re also here to help you access your records when you need them.
Your Right to Access
You have the right to review and receive a copy of your medical record. Our team is highly trained in electronic health record management and committed to protecting your privacy. For more details, see our Notice of Privacy Practices (NPP), which explains how we use and share your information.
How to Request Your Records
To request a copy of your medical record, please complete the Authorization for Disclosure of Protected Health Information (PHI) form.
You Can Get this Form By:
- Downloading it here: ADM0702 Authorization for Disclosure of PHI
- Picking up a copy from the Health Information Department, 2nd Floor, Northern Montana Hospital, during regular business hours
- Calling the Health Information Department at (406) 262-1230 to request a copy by mail or fax
- Contact the Health Information Department via email at NMHCHIM@nmhcare.org to request the form
If you pick up records in person, please bring a current photo ID (driver’s license, state-issued ID, or passport). If mailed, records will be sent to the address listed on your form.

Costs for Medical Records
Records on a CD: $10 flat fee
Records 5 pages or fewer: no charge
Records over 5 pages: $0.50 per page
Please note: exemptions may apply in certain situations, such as cases involving abuse, neglect, or minor consent laws. For example, Montana minors may consent to certain treatments without parental involvement, and in those cases records are handled with additional privacy protections.
